It's possible to start the process by using a culture but I agree that the nomal is to just smash the cabbage, pack it and salting it. There have to be enough juices to cover the cabbage (the juices have to cover the weight stones).
The top of the jar acts as an airlock when filled with water. Gasses can still escape.
