Well, I decided that it was time to start taking notes since things are starting to get more involved and we are making some more difficult cheeses.  I have created a PDF "form" that can be filled in on your computer or printed as well.  If you have Adobe Pro you can use the save as feature and save the form after it is filled out, other wise it will only let you print. 

The form is a combination a couple I have seen on here and some of it is beyond my skill level but I get there.  If it helps anyone,  great!

I realized that it had a date on there and I have taken that off as well as changed the field where you enter how many gallons of milk you are suing.  Now if you dont want to fill out he form on your computer you can just print it out as a blank form.   Hope this helps.
